2021 ASA License collection

 

Dear Bellville Athletic Club member

The 2021 license
numbers can be collected at the club this Thursday (13 May) between 17h30
and 19h00. 



Please remember, proof of payment must be emailed to bakentries@gmail.com by 12 noon today to leave
sufficient time to have your license number allocated to you. Only paid up
members (license and membership fee) will be allowed to collect their
license.

 

When making payment, use your surname and "2021" as payment
reference. Membership and license fee information can be found on the “Welcome
to Bellville Athletic Club 2021” document. Download the “Welcome to Bellville
Athletic Club 2021” form here. 



NB! Members also need to update their ASA profile and should
email or bring with the following documents:

• Signed ASA
  membership 2021 form, alternatively they can print the ASA form from their
  profile on the ASA website (autogenerated form, nothing needs to be completed)
  and;
• the “Welcome to
  Bellville Athletic Club 2021” rules form (also signed).

If you are unable
to collect your license number, someone else may collect on your behalf, provided
that:

• you have mailed
  your proof of payment by no later than 12pm on Wednesday and;
• the person
  collecting on your behalf brings the 2 completed forms mentioned above;
